How about Peter Chen's original paper on the subject?  Perhaps this does not
qualify as a survey, however.  Anyway, it can be found in ACM's Transactions on
Database Systems (vol 1, no 1, March '76, pp 9-36) as "The Entity-Relationship
Model:  Toward a Unified View of Data."
