I found out the problem a bit more specifically. If you have just went
up a flight of stairs and are standing in the staircase of the next level,
you can not shoot a fireball properly. Feature or bug?

BTW, is anyone from FTL listening? How do I get in touch with them to
send in bug/oddness reports?
