I had the same problem.  I kept on trying different interleaves and it finally 
worked.  Another suggestion is to try using HDFORMAT or HDFORMAT1.1.  I don't
know if they are shareware or not... but I do have them.  They work just as
as hacker.

|Same problem occurs when running from Prodos 8 booted on a 3.5 drive, except
|that it quits to the obnoxious Prodos quit routine instead of the finder.

If none of that helps... Try calling your local Apple dealer, the Makers of
the HD, or probably Seagate (which is the most commonly used drive (or so
i've found) in the pre-packaged SCSI systems).
