I have experienced similar errors when a drive came to a crashing halt.
The DISK command reports IO errors from the last time the FS was booted.
Each time it is booted the IO errors reset to 0.  Actually,  you may
have similar errors when the controller and drive "flake out".  I think
there are times when the controller "gets lost" during heavy activity, like
during a backup.
