I had the same problem : "Sector 0 Corrupt, Disk Unuseable" or something
like that. (IBM PS/2 50Z running DOS 4.0)  I was formatting many generic
3.5" diskettes, and about 20 worked fine. I had 5 more to do. All gave
the same error. I took each disk and simply rotated the disk inside the
sleeve manually. Retried it, and they work fine. Don't know if this
helps, but it works for me.
