Yep, Ann, a doctor's certificate of disability -is- required.  I had to
have one for the AT&T TDD service and the PCPursuit service.

Ya know who is giving me fits right now?  GREYHOUND BUS COMPANY. 
Sometimes they are so over-protective and sometimes they just don't
care.  I just got back from a sports-related Ski Race and got super
help on my trip there and -no- help on my return trip.
