We've seen a sporadically-recurring problem under SunOS 4.0.3...  simply
logging in will produce:

 bmers63 vmunix: assertion failed: vp->v_stream, file: ../../os/s tr_io.c, line: 1756
 bmers63 vmunix: panic: assertion failed
 bmers63 vmunix: syncing file systems... [6] 1 [6] [4] [1] done
 bmers63 vmunix: dumping to vp f295a8c, offset 44168
 bmers63 vmunix: 1022 total pages, dump succeeded
 bmers63 vmunix: rebooting...

It's not consistent; it may not recur for weeks.  We're a binary-only
site; the problem is being reported to SUN as well, but I'm hoping that
someone has already dealt (successfully?) with this problem.
