If an \item with several \subitems get pagebroken somewhere in the \subitems, no " (Continued)" is written on the next page. 2. The index is set in two columns but the columns are not balanced on the last page, ie the left column always goes all the way down, even if the right column is just a few lines. Has anyone written a .sty which fixes these things? I know there are hints in The TeXbook for doing exactly this, but I'd rather not port to LaTeX if it already has been done.
